Member Evaluation Survey

Thank you for putting time aside to complete and provide feedback on TimeSlips’ new evaluation tool! The 3-5 minutes you spend today will help our programs continue to improve and make the case to new funders. Below are a few key definitions about TimeSlips:

• TimeSlips programming: We are referring to the TimeSlips techniques you have learned, such as: Asking a Beautiful Question (an open-ended question with no right or wrong answers), Yes, And… (radically accepting what your Storyteller offers and adding to it), and/or Proof of Listening (validation of the Storyteller’s response).


• Storyteller(s): We are referring to the person or people you use TimeSlips with. This may be someone living with dementia, an older adult, or anyone that you have chosen to use creative engagement with. The Storyteller could be one person or a group of Storytellers.

All responses will be kept confidential and TimeSlips will not link individuals to the responses provided. In other words, we welcome any and all feedback to accurately understand our impact!
